About the Role
An exciting opportunity is available for a CDM Nurse (RN or EEN) to join our Ballarat medical centre.
Working closely with GPs and the wider healthcare team, you will play a key role in delivering high-quality chronic disease management and preventative healthcare services. You will support patients living with chronic conditions through care planning, health assessments, education and ongoing health coaching, helping them achieve better long-term health outcomes.

Key Responsibilities
• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
• Conduct health assessments, including 75+ Health Assessments, Heart Health Checks, Diabetes Risk Assessments, ATSI Health Assessments, and Veteran, Refugee and Intellectual Disability Health Assessments.
• Support patients in setting SMART goals and developing action plans to manage chronic health conditions.
• Provide ongoing nurse monitoring, education and health coaching.
• Coordinate care between GPs, specialists and allied health providers to deliver holistic patient care.
• Perform clinical services including ECGs, Spirometry, ABI testing and Urinalysis.
• Identify eligible patients for chronic disease management programs and preventative healthcare services.
• Maintain accurate clinical documentation and Medicare compliance.
• Build strong relationships with GPs and practice staff to promote preventative healthcare and increase patient engagement.
• Support and educate nursing and administrative teams on chronic disease management processes.

About You
To succeed in this role, you will have:
• Current unrestricted AHPRA registration as a Registered Nurse or Enrolled Nurse.
• Previous nursing experience within general practice, primary healthcare or a similar clinical environment.
• Experience in Chronic Disease Management or a strong interest in preventative healthcare.
• Strong communication and patient engagement skills.
• Excellent organisational and time management abilities.
• A proactive, patient-centred approach to care.
• Knowledge of Medicare-funded health assessments, care planning and chronic disease management processes is highly regarded.
• Unrestricted Australian working rights.
• The ability to provide evidence of vaccination history and immunity records.
