Te Mahi | About the Role

As a Community Nurse, you will provide high-quality, evidence-based nursing care in people’s homes and other community settings. You’ll take ownership of your caseload while working closely with our clinical team and a wider network of healthcare professionals.

Your day-to-day work may include:

• Comprehensive nursing assessments and clinical reviews
• Developing, implementing and evaluating individualised care plans
• Providing acute and chronic wound management, including complex wounds and burns
• Catheter care and management
• Supporting complex and serious injury service users
• Medication management and other clinical interventions
• Health education for service users, families and whānau
• Providing clinical guidance, training and competency support to Support Workers
• Working collaboratively with GPs, ACC Case Managers, NASC, allied health professionals and other community providers
• Supporting a wraparound model of care across Healthvision
• Maintaining strong professional relationships with referrers and healthcare partners
• Maintaining high standards of clinical documentation, safety and quality

Depending on your experience and interests, there are opportunities to build your skills in areas such as:

• Compression and Doppler therapy
• Negative Pressure Wound Therapy
• Catheter and continence management
• Complex/serious injury care
• IV therapy
• Pain management
• Skin integrity and pressure injury prevention
• Clinical education and staff competency development

You'll receive comprehensive orientation, ongoing clinical support, supervision and professional development so you can feel confident in your role.

About You

You’ll be a Registered Nurse or Enrolled Nurse who combines strong clinical skills with genuine compassion.

You enjoy building relationships with people, can work confidently on your own, and are equally comfortable collaborating with a wider healthcare team.

Ideally, you will have:

• At least 2 years' post-registration nursing experience in acute, surgical, community or practice nursing
• Strong wound management experience
• Experience with catheter care
• Excellent clinical assessment and decision-making skills
• Strong communication and relationship-building skills
• Excellent organisation and time-management skills
• A proactive, positive and solutions-focused approach
• The ability to work autonomously while knowing when to seek support
• A commitment to culturally safe and person-centred care
• An understanding of and commitment to the principles of Te Tiriti o Waitangi

Experience in complex wounds, burns, serious injury, compression therapy, Negative Pressure Wound Therapy, IV therapy or clinical education would be highly advantageous.

Essential Requirements

• Current NZ Nursing Council practising certificate
• Current Full or Restricted New Zealand driver's licence
• Access to a reliable vehicle
• Clean Police Clearance
• Eligibility to legally work in New Zealand
• Flexibility to travel throughout the Bay of Plenty
