About the role
This position is based across cohealth's CBD services at 244 Flinders Street, which provide integrated primary health care and harm reduction support to people who use drugs, with a strong focus on improving health and social outcomes for individuals experiencing complex needs.Please refer to the Position Description to know more about the role.
Your duties include:
• Provision of high quality ethical, evidence-based, flexible, professional goal-directed and client-centred services, with focused psychological strategies
• Provide short term casework, assessment and care planning
• Assess psychosocial needs and support clients through advocacy, service navigation, and care coordination
• Work collaboratively within multidisciplinary and cross-sector teams

Our ideal candidate:
• Tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
• Sound understanding of trauma-informed, recovery-oriented, and human rights-based practice
• Proven experience delivering short-term casework, psychosocial assessment, and care planning with individuals experiencing complex social and mental health needs
• Ability to provide advocacy, secondary consultation, and service navigation support within multidisciplinary settings
