Community-Based PHN Nurse Coordinator

Under direction of Head Nurse and/or Program management, this nursing position coordinates outreach to settings with high burden of infectious diseases to support infection control, case and contact investigations, and care navigation. The supervising nurse is a primary liaison to settings such as homeless shelters, carceral facilities, state hospitals, and other congregate settings. The supervising nurse will respond to cases of reportable infectious diseases identified in residents of these settings and will coordinate appropriate contact investigations and infection control protocols. The supervising nurse may support direct patient care through care navigation, supervision of directly observed therapy, administration of post-exposure prophylaxis, or other measures. The supervising nurse will also maintain strong relationships with facility leadership and guests in order to support the development and implementation of proactive infection prevention strategies. The supervising nurse may supervise and support other staff, including community health workers and staff nurses providing tuberculosis case management services to guests of homeless shelters or other facilities.

Duties

• Plays a leadership role in the investigation and follow-up of communicable disease cases/outbreaks or other situations of public health concern in community settings such as homeless shelters, carceral facilities, and other congregate settings.

• Performs communicable disease surveillance, prevention and control measures according to protocols for cases/outbreaks as assigned. In conjunction with other Division staff, identifies patterns of communicable diseases and related risk factors, and provides health education and outreach as needed for communicable disease prevention and control.

• Investigates and provides care and follow-up of contagious diseases, including directly observed therapy, administration of post-exposure prophylaxis, and other measures.Assists with development of data collections tools and ensures complete and accurate collection of data.

• Supervises subordinates as required. Assists in orientation of new staff, program visitors and in staff development programs.

• Identifies or develops resources as needed for case or outbreak follow-up.

• Provides nursing support for CDC Division activities, including vaccine and other clinics or other projects or activities as needed.

• Provides nursing services at the TB Clinic, including, patient assessment, patient/family/significant other education, medication monitoring, and skin testing.

• Participates in special projects and initiatives as requested and acts as clinical back-up for special projects.

• Participates in staff conferences, research activities, meetings with community based or other agencies as requested.

• Plays a key role in communicable disease related epidemiologic activities.

• Plays a key role in providing education and outreach on communicable disease issues to health care providers, agencies, and community groups throughout the city, with a focus on homeless shelters, carceral facilities, and other congregate settings. Acts as a resource on communicable disease related issues for division staff, providers, and community groups.

• Maintains strong relationships with key partners providing services for individuals in homeless shelters, carceral facilities, and other congregate settings.

• May be required to perform duties of other CD or TB nurses.

• Performs other duties as required.

• Works to help BPHC advance as an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.

Qualifications

• BSN with nursing license in MA
• Bachelor's degree in Nursing or related field required (10 or more years of relevant clinical experience may substitute for bachelor's degree)
• Master's Degree in nursing, education, or public health preferred
• 3-5 years public health experience required
• Prior communicable disease tuberculosis experience preferred.
• Demonstrated supervisory skills required.
• Strong assessment, planning, evaluation and intervention skills. Excellent teaching and counseling abilities and written and verbal communication skills.
• Demonstrated leadership and
  organizational abilities. Excellent written and verbal communication skills.
• Demonstrated ability to work with diverse populations.
• Prior phlebotomy experience preferred or willingness to learn.
• Prior experience with EPIC or other electronic medical record preferred.
• Bilingual (Spanish, Portuguese, Cape Verdean Creole, Haitian Creole, Vietnamese, or Chinese) preferred.
• Valid Mass. drivers license and daily access to working automobile.
• Knowledge of or willingness to learn about the role of public health in addressing racism, the social determinants of health, and inequities in health outcomes as well as strategies to advance racial justice and health equity.
• Commitment to health equity and racial/social justice issues.

40 hrs/week 9-5:30 or alternate shift as determined by the division.

Pay Range

$1,666.71 - $2,345.20 / wk
