Summary of the Position: As a Wraparound Facilitator, you will partner with youth experiencing serious emotional and behavioral challenges and their families to build individualized plans that promote stability, resilience, and long-term success. Using a family-centered, strengths-based, and community-driven approach, you will help families identify resources, coordinate supports, and achieve their goals. Services are provided in homes and community settings, allowing you to work directly where families live, learn, and thrive.
- Facilitate the Wraparound process by helping youth and families identify strengths, build natural support networks, and create individualized plans for success.
- Provide intensive case management and care coordination services that empower families, promote resilience, and support positive outcomes.
- Lead and facilitate Wraparound Team meetings, bringing together family members, service providers, schools, and community supports to ensure a collaborative approach.
- Build strong, trusting relationships with youth and families while advocating for their needs and connecting them to community resources.
- Maintain timely and accurate documentation and communicate progress, barriers, and outcomes with supervisors, team members, and referral sources.
- Deliver services in accordance with Medicaid requirements, organizational standards, and state and federal regulations.
Qualifications/Requirements:
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field required.
- Passion for supporting children, youth, and families through a strengths-based, family-centered approach.
- Strong communication, facilitation, relationship-building, and organizational skills.
- Proficiency with Microsoft 365; experience with electronic health records (EHR) systems preferred.
- Reliable transportation, valid driver's license, and current auto insurance coverage required.
- Bilingual (Spanish/English) candidates are encouraged to apply.
- Flexible hours; Evenings and weekends may be required
