Responsibilities

Provide direct nursing care, medications, and treatments as prescribed.
Monitor resident condition and report changes to the care team.
Accurately document resident care and interventions.
Collaborate with nurses, physicians, therapists, and other team members.
Follow safety, infection prevention, and regulatory standards.
Deliver compassionate, patient-centered care.

Qualifications

Active, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
Graduate of an accredited Practical Nursing program.
Experience in skilled nursing, long-term care, rehabilitation, or post-acute care preferred.
Strong clinical assessment, medication administration, and documentation skills.
Strong communication and teamwork skills.
